Abstract:【Objective】 To explore the application effect of palliative care centered on dignity therapy in elderly terminally ill patients. 【Methods】 A total of 88 elderly terminally ill patients who received palliative care in our hospital from January 2020 to February 2025 were selected and divided into a control group (receiving routine palliative care intervention) and an observation group (receiving palliative care intervention based on dignity therapy) with 44 cases in each group using the random number table method. The psychological distress level [Distress Management Screening Measure (DMSM) score], sense of dignity [Patient Dignity Inventory (PDI) score], self-esteem level [State Self-Esteem Scale (SSES) score], fear status [Fear of Progression Questionnaire (FoP-Q) score] and quality of end-of-life care [Palliative Outcome Scale (POS) score] were compared between the two groups. 【Results】 After intervention, the DMSM scores and PDI scores of both groups were lower than those before intervention, and the observation group was lower than the control group, with statistically significant differences (P<0.05). The scores of behavioral self-esteem, social self-esteem and total score of both groups were higher than those before intervention, and the observation group was higher than the control group, with statistically significant differences (P<0.05). The scores of emotional response, occupational impact, limited autonomy, intimate relationship/family support, anxiety coping and total score of both groups were lower than those before intervention, and the observation group was lower than the control group, with statistically significant differences (P<0.05). The scores of pain management, symptom relief, emotional anxiety, family emotional burden, information acquisition and communication, support level, self-cognition, sense of life meaning, time consumption and affair handling in the observation group were lower than those in the control group, with statistically significant differences (P<0.05). 【Conclusion】 Palliative care based on dignity therapy can effectively alleviate the psychological distress and fear of elderly terminally ill patients, and improve their sense of dignity, self-esteem level and quality of end-of-life care.
