门冬胰岛素辅助地特胰岛素治疗妊娠期糖尿病患者的临床疗效

摘要 【目的】探讨门冬胰岛素辅助地特胰岛素治疗妊娠期糖尿病(GDM)患者的临床疗效。【方法】选取2022年1月至2024年12月本院收治的150例GDM患者,按1∶1随机分为观察组和对照组,每组75例。对照组患者接受门冬胰岛素治疗,观察组患者在对照组的基础上联合地特胰岛素治疗。比较两组临床疗效、血糖[空腹血糖(FPG)、餐后2 h血糖(2hPG)]水平、胰岛相关激素指标和脂肪因子[C肽、内脂素(VF)、瘦素(LP)、脂联素]水平、胰岛细胞功能指标[空腹胰岛素(FINS)、稳态模型胰岛 β 细胞分泌指数(HOMA-β)]、胰岛素抵抗指数(HOMA-IR)。【结果】观察组总有效率高于对照组(P<0.05)。治疗后,观察组FPG、2hPG、VF、LP、FINS水平及HOMA-IR低于对照组(P<0.05),C肽、脂联素、HOMA-β高于对照组(P<0.05)。【结论】门冬胰岛素辅助地特胰岛素治疗GDM可显著提高临床疗效,改善血糖状况,调节胰岛相关激素水平,增强胰岛细胞功能。

Abstract：【Objective】 To explore the clinical efficacy of insulin aspart combined with insulin detemir in the treatment of patients with gestational diabetes mellitus (GDM). 【Methods】 A total of 150 patients with GDM admitted to our hospital from January 2022 to December 2024 were selected and randomly divided into the observation group and the control group at a ratio of 1∶1, with 75 cases in each group. The control group was treated with insulin aspart alone, while the observation group was treated with combined insulin aspart and insulin detemir on the basis of the control group. The clinical efficacy, blood glucose levels [fasting plasma glucose (FPG), 2-hour postprandial blood glucose (2hPG)], islet-related hormone and adipokine levels [C-peptide, visfatin (VF), leptin (LP), adiponectin], islet cell function indicators [fasting insulin (FINS), homeostasis model assessment of β-cell function (HOMA-β)] and homeostasis model assessment of insulin resistance (HOMA-IR) were compared between the two groups. 【Results】 The total clinical effective rate of the observation group was higher than that of the control group (P<0.05). After treatment, the levels of FPG, 2hPG, VF, LP, FINS and HOMA-IR in the observation group were lower than those in the control group (P<0.05), while the levels of C-peptide, adiponectin and HOMA-β were higher (P<0.05). 【Conclusion】 The combination of insulin aspart and insulin detemir can significantly improve the clinical efficacy, optimize blood glucose status, regulate the levels of islet-related hormones and enhance islet cell function in the treatment of GDM.
