Abstract：【Objective】To explore the effect of transitional care model on the rehabilitation of elderly patients after total hip arthroplasty. 【Methods】From November 2020 to August 2022, 136 patients with artificial total hip arthroplasty admitted to the Department of Traumatology and Arthroplasty of our hospital were selected as the study subjects and divided into experimental and control groups using the random number table method, with 68 patients in each group. In the control group, conventional treatment and care were used, while in the experimental group ( the observation group), a transitional care model was applied. The Harris Hip Function Score and Barthel Index were compared between the two groups. 【Results】There was no statistically significant difference in the preoperative Harris hip function scores between the two groups of patients (P>0.05). The Harris hip function scores of the two groups of patients at each time point after discharge were higher than those before surgery, and the scores in the observation group was higher than those in the control group, and the difference was statistically significant (P<0.05). The Barthel index scores of the two groups of patients at each time point after discharge were higher than those before surgery, and the scores in the observation group was higher than those in the control group, and the difference was statistically significant (P<0.05).【Conclusion】The transitional care model can significantly improve the functional rehabilitation and self-care ability of the hip joint in the elderly patients after total hip arthroplasty.
