Abstract  【Objective】 To explore the therapeutic efficacy of hyperbaric oxygen therapy combined with edaravone on acute cerebral infarction and its impacts on cerebral hemodynamics and neurological function of patients.【Methods】A total of 106 patients with acute cerebral infarction were randomly divided into the observation group and the control group, with 53 patients in each. The control group received conventional treatment, while the observation group was treated with hyperbaric oxygen combined with edaravone on top of the conventional treatment. The treatment outcomes, cerebral hemodynamics parameters [mean blood flow (Qmean), mean blood flow velocity (Vmean), peripheral resistance (R), and dynamic resistance (DR)], neurological function indicators [neuron-specific enolase (NSE), S100-beta protein (S100-β), myelin basic protein (MBP), and glial fibrillary acidic protein (GFAP)] levels, and adverse reactions were compared between the two groups before treatment and 2 weeks after treatment.【Results】 The total effective rate of the observation group was significantly higher than that of the control group (P<0.05). After 2 weeks of treatment, Qmean and Vmean levels significantly increased, while R and DR levels significantly decreased in both groups compared to before treatment (P<0.05). And the observation group showed significantly higher levels of Qmean and Vmean, but lower levels of R and DR compared to the control group (P<0.05). The levels of NSE, S100-β, MBP, and GFAP significantly decreased after 2 weeks of treatment in both groups (P<0.05), with the observation group showing significantly lower levels than the control group (P<0.05). There was no significant difference in the total incidence of adverse reactions between the two groups (P>0.05).【Conclusion】 The combination of hyperbaric oxygen and edaravone in treating acute cerebral infarction shows better efficacy, significantly improves cerebral hemodynamics, effectively restores neurological function, and has high safety, making it worthy of clinical promotion.
