The Clinical Value of miRNA-208b in Predicting Left Ventricular Remodeling after AMI

Abstract  【Objective】To explore the clinical value of miRNA-208b (miR-208b) in predicting left ventricular remodeling after acute myocardial infarction (AMI). 【Methods】A total of 167 AMI patients admitted to our hospital from November 2018 to September 2020 were selected, and they were divided into a remodeling group (n=62) and a non remodeling group (n=105) based on whether there was left ventricular remodeling during follow-up. The clinical indicators and expression of miR-208b, as well as the correlation between miR-208b and left ventricular remodeling, were analyzed in two groups of patients. The receiver operating characteristic (ROC) curve analysis was used to evaluate the value of miR-208b in left ventricular remodeling in AMI patients. 【Results】The final left ventricular end diastolic volume (LVEDV), left ventricular end diastolic volume increase (LVEDV), low-density lipoprotein cholesterol (LDL-C), cardiac troponin I (cTnI), and brain natriuretic peptide (BNP) levels of patients in the reconstruction group were higher than those in the non reconstruction group, and the expression level of miR-208b was lower than that in the non reconstruction group, with statistical significance (P<0.05). Pearson correlation analysis showed that miR-208b was negatively correlated with BNP and final LVEDV (P<0.05), and positively correlated with cTnI (P<0.05). ROC curve analysis showed that miR-208b had a high value in predicting left ventricular remodeling after AMI, with an area under the curve (AUC) (95%CI) of 0.80(0.73-0.87) and an optimal diagnostic threshold of 3.17. Binary logistic regression analysis showed that miR-208b, cTnI, LVEDV, and BNP were closely related to left ventricular remodeling (P<0.05). 【Conclusion】miR-208b is a potential clinical indicator for diagnosing left ventricular remodeling after AMI, and clinical physicians can estimate the risk of left ventricular remodeling in AMI patients based on miR-208b expression.
