Effect of Abdominal Electrical Stimulation on Pulmonary Infection and Airway Clearance in Patients with Severe Tracheostomy |

Abstract 【Objective】To explore the effect of abdominal electrical stimulation on the incidence of pulmonary infection and airway clearance ability in patients with severe tracheostomy.【Methods】Eighty patients who underwent tracheostomy for severe conditions were admitted to our hospital from February 2021 to February 2022. They were randomly divided into a control group and an observation group,each consisting of 40 patients. The control group received conventional treatment and early training,while the observation group additionally received abdominal electrical stimulation. Both groups were treated for two weeks. The incidence of pulmonary infection,average hospital stays,time of tracheal tube retention,and parameters like Forced Expiratory Volume in 1 second (FEV1),Forced Vital Capacity (FVC),FEV1/FVC ratio,diaphragmatic mobility,diaphragmatic thickening fraction,sputum production,abdominal muscle thickness,and peak cough flow were compared.【Results】The observation group showed lower rates of pulmonary infection,shorter average hospital stays,and shorter tracheal tube retention time compared to the control group (P<0.05). After treatment,the observation group had higher values of FEV1,FVC,FEV1/FVC ratio,diaphragmatic mobility,diaphragmatic thickening fraction,and sputum production than those before treatment. and the control group after treatment (P<0.05). No significant difference was observed in the thickness of external oblique and internal oblique abdominal muscles before and after treatment in both groups (P>0.05). The observation group showed a significant increase in peak cough flow after treatment compared to before treatment and the control group after treatment (P<0.05).【Conclusion】Abdominal electrical stimulation in patients with severe tracheostomy can increase diaphragmatic mobility,improve cough ability and sputum discharge,enhance airway clearance ability,thereby reducing the incidence of pulmonary infections and shortening the duration of tracheal tube retention and hospital stay.
