Expression of Peripheral Blood Toll-like Receptor 4 and its Correlation with Urinary Microalbumin Level in Chronic Glomerulonephritis

Abstract  【Objective】 To investigate the expression of peripheral blood Toll-like receptor 4 (TLR4) in chronic glomerulonephritis (CGN) and its relationship with urinary microalbumin (UMA) level. 【Methods】 A total of 113 CGN patients admitted to Anyang Hospital of Traditional Chinese Medicine,including 77 patients with mild renal insufficiency,24 patients with moderate renal insufficiency and 12 patients with severe renal insufficiency,were enrolled in the observation group. Another 59 healthy volunteers who underwent physical examination in our hospital during the same time period were selected as the control group. The expression of renal function indexes such as TLR4 in peripheral blood and UMA in the observation group and the control group were detected,then the correlation between renal function index TLR4 and UMA in the observation group was analyzed. 【Results】 The levels of CD19+TLR4+ cells and CD3+TLR4+ cells in the severe renal insufficiency group were higher than those in the moderate renal insufficiency group and the mild renal insufficiency group (P<0.05). The levels of CD19+TLR4+ cells and CD3+TLR4+ in the moderate renal insufficiency group were higher than those in the wild renal insufficiency group (P<0.05). The levels of CD19+TLR4+ cells and CD3+TLR4+ cells in the observation group were higher than those in the control group (P<0.05). The levels of UMA,retinol binding protein (RBP) and transferrin (TRF), cystatin C,creatinine,blood urea nitrogen and uric acid in the observation group were higher than those in the control group (P<0.05). Pearson correlation analysis showed that the level of CD19+TLR4+ cells in the observation group was positively correlated with UMA,RBP,and cystatin C (P<0.05),and the level of CD3+TLR4+ cells was positively correlated with UMA (P<0.05). 【Conclusion】 The peripheral blood TLR4 expression of CGN patients is abnormally high,and the expression level of TLR4 is closely related to renal function indicators such as UMA. The higher the expression level of TLR4 is,the worse the renal function of CGN patients will be.
