Abstract 【Objective】 To evaluate the correlation between the relative lymphocyte count (L%) and the 3-year mortality in elderly patients with COPD.【Methods】 A total of 69 patients with chronic obstructive pulmonary disease (COPD) in our hospital from August 2015 to August 2016 were selected for the study. Patients were divided into two groups according to the relative lymphocytes count. Group A was the relative lymphocyte count ≤ 20% and the group B was the relative lymphocyte count > 20%. The general data of the two groups were collected, all patients were followed up for 3 years after discharge. Thus the general data of the two groups were compared and the relationship between the relative lymph count and the 3-year mortality rate was analyzed. 【Results】 Among 69 patients, 27 (39.13%) patients had the relative lymphocyte count ≤20% (group A), and 42 (60.87%) patients had the relative lymphocyte count >20% (group B).The 3-year mortality rates of patients in the group A and group B were 81.48%(22/27)and 57.14%(24/42),respectively (P<0.05). The survival curve analysis showed that the mortality of patients with relative lymphocytes count ≤ 20% was higher; And the difference was statistically significant (P<0.05). After adjusting for age and gender the risk of death was 1.88 [95% confidence interval CI:1.21~2.56,P<0.01].【Conclusion】 Lower relative lymphocyte count is associated with higher mortality in elderly patients with severe COPD. This result suggests that relative lymphocyte count can predict the prognosis of elderly patients with severe COPD.
