外科手术治疗颅内海绵状血管瘤伴癫痫的安全性及效果观察

Abstract 【Objective】 To investigate the safety and clinical effect of surgical treatment of intracranial cavernous hemangioma with epilepsy. 【Methods】 A total of 76 cases of intracranial cavernous hemangioma with epilepsy in our hospital from January 2013 to August 2017 were selected. The postoperative clinical effect, postoperative neurological function recovery and clinical complications were recorded.【Results】 76 patients were followed up postoperatively. At the last follow-up, Engel classification was grade Ⅰ in 41 cases (53.95%), grade Ⅱ in 19 cases (25.00%), grade Ⅲ in 12 cases (15.79%), grade Ⅳ in 4 cases (5.26%). The proportion of patients with course of disease less than 2 years and surgical method of complete resection + cauterization were 78.79% and 65.91%, respectively, which were significantly higher than those of patients with course of disease ≥ 2 years and complete resection (P<0.05).Logistic regression analysis showed that duration of disease ≥ 2 years was the risk factor of Engel grade Ⅰ (OR=1.956, P<0.05).Complete resection + cauterization were the protective factors of Engel grade Ⅰ (OR=0.643, P<0.05).【Conclusion】 Surgical treatment of intracranial cavernous hemangioma with epilepsy has good effect and safety, worthy of clinical application.
