Abstract 【Objective】To detect the effects of videoassisted thoracoscopic surgery (VATS), minimally invasive incision and conventional thoracotomy on perioperation pulmonary function in patients undergoing pulmonary lobectomy. 【Methods】Retrospective analysis of Our hospital's Cardiothoracic Surgery Department, October 2011 to July 2014, were treated 100 cases of lobectomy patients with clinical data, according to the surgical route divided into VATS group (30 cases), minimally invasive group (34 cases) , thoracotomy group (36 cases). compared Three groups of changes in lung function in patients before and after surgery were compared in retrospective analysis. 【Results】Preoperative, three groups of patients with differences in FVC, FEV1, MVV, PEF values of three groups of patients were measured value difference was not statistically significant (P>0.05); postoperative, 1,3,5,and 7d of FVC, FEV1, MVV, PEF measured values were measured and the differences were statistically significant (P<0.05). FVC, FEV1, MVV,and PEF measurements in the VATS group was significantly higher than that of minimally invasive thoracic surgery group and thoracotomy group (P<0.05); minimally invasive thoracic surgery group and the group the differences between values in the latter two groups were was not statistically significant (P>0.05). Preoperative and 1, 3d postoperative 1,3d the three groups of patients had no statistically significant differences in with PaO2 and PaCO2 measured value difference was not statistically significant (P>0.05),however,5 and 7d postoperative,PaO2 values of the VATS group were significantly higher than those of the other two groups. the VATS group PaO2 after the first minimally invasive group 5,7d and thoracotomy group, PaCO2 measured values were significantly lower than those of the other two groups minimally invasive thoracic surgery group and group (P<0.05). 【Conclusion】Implemented using The use of VATS lobectomy has a smaller effect on lung function and blood gas index of patients than the on the more minimally invasive and conventional thoracotomy lobectomy. small, The differences between minimally invasive and conventional thoracotomy lobectomy for lung function and blood gas index the impact is insignificant.
