Abstract:【Objective】To explore the clinical effect of unilateral craniotomy in the treatment of frontal lobe contusion and laceration combined with intracranial hematoma.【Methods】From June 2015 to June 2017, 110 patients with frontal lobe contusion and laceration combined with intracranial hematoma in our hospital were divided into unilateral craniotomy (observation group,n=60) and bilateral craniotomy (control group, n=50). The operation time, blood transfusion volume, hospitalization time, Glasgow Coma score (GCS) and Glasgow pre-treatment were compared between the two groups The changes of serum NSE, S100 β protein and lactate were observed before and after operation. 【Results】 The operation time, blood transfusion volume and hospitalization time in the observation group were significantly lower than those in the control group (P<0.05). One week after operation, the GCS score of the observation group was significantly higher than that of the control group (P<0.05). Three months after operation, the prognosis of the patients in the observation group was 66.67%, which was significantly higher than that in the control group (46.00%) (P<0.05). There was no significant difference in serum NSE, S100 β protein and blood lactate concentration between the two groups on the first day after operation (P>0.05); the serum NSE, S100 β protein and blood lactate concentration in the observation group were significantly lower than those in the control group on the first week after operation (P<0.05).【Conclusion】Compared with bilateral craniotomy, the patients with frontal lobe contusion combined with intracranial hematoma have less trauma and better postoperative recovery.
