摘要【目的】 探讨CACUL1在乳腺癌中的表达及其与心理社会因素的相关性。【方法】 选取2014年6月至2018年3月期间于本院行乳腺癌根治术的169例乳腺癌患者为研究对象,采用荧光实时定量聚合酶链反应(qPCR)、Western blot法与免疫组织化学染色(IHC)比较癌组织和癌旁组织CACUL1的表达水平,根据IHC结果将乳腺癌患者分为CACUL1高表达组(n=103)和低表达组(n=66)。比较两组的基线资料、汉密尔顿焦虑量表(HAMA)、汉密尔顿抑郁量表-17项(HAMD-17)、社会支持评定量表(SSRS)、艾森克人格问卷简版(EPQ)、乳腺癌病人生命质量测定量表(FACT-B)等评分情况。【结果】 ①乳腺癌组织的CACUL1 mRNA(2.6±0.8 vs. 1.0±0.1,P<0.001)、蛋白表达量(1.9±0.5 vs. 1.0±0.2,P<0.001)、IHC评分(55.6±20.9 vs. 18.6±7.2,P<0.001)均显著高于癌旁组织。②高表达组的年龄、月收入≤5000元、分期为Ⅲ~Ⅳ期的比例均显著高于低表达组,差异有统计学意义(P<0.05)。③高表达组的HAMA评分、HAMD-17评分、EPQ的神经质、精神质评分、FACT-B的社会/家庭状况、情感状况评分及总分均显著高于低表达组,SSRS量表的主观支持、对社会支持的利用度评分及总分均显著低于低表达组,差异有统计学意义(P<0.05)。【结论】 乳腺癌组织的CACUL1表达水平明显高于癌旁组织,且其与心理社会因素有关,高表达者的不良情绪更明显,社会支持水平与生活质量更低,肿瘤分期更差。
Abstract:【Objective】 To analyze the expression of CACUL1 in breast cancer and its correlation with psychosocial factors. 【Methods】 A total of 169 patients with breast cancer who underwent radical mastectomy in our hospital from June 2014 to March 2018 were enrolled for the study. The expression level of CACUL1 was compared between cancer tissues and adjacent tissues through quantitative real-time polymerase chain reaction (qPCR), Western blot and immunohistochemical staining (IHC). According to the result of IHC, patients were divided into the high expression group (n=103) and the low expression group (n=66). Baseline data, Hamilton Anxiety Scale (HAMA), Hamilton Depression Scale-17 (HAMD-17), Social Support Rating Scale (SSRS), Eysenck Personality Questionnaire (EPQ) and FACT-B were compared between two groups. 【Results】 ⑴The CACUL1 expression of mRNA (2.6±0.8 vs. 1.0±0.1,P<0.001), protein (1.9±0.5 vs. 1.0±0.2,P<0.001) and IHC score (55.6±20.9 vs. 18.6±7.2,P<0.001) in breast cancer tissues were significantly higher than those in adjacent tissues. ⑵Age, monthly income < 5000 yuan and the proportion of stage III-IV in the high expression group were significantly higher than those in low expression group (P<0.05). ⑶The scores of HAMA, HAMD-17, neuroticism and psychoticism of EPQ, social/family status, emotional status and total scores of FACT-B in the high expression group were significantly higher than those in the low expression group, and the scores of subjective support, utilization of social support and total score of SSRS were significantly lower than those in the low expression group (P<0.05). 【Conclusion】 The expression level of CACUL1 in breast cancer tissues is significantly higher than that in adjacent tissues, and it is related to psychosocial factors. Patients with high expression of CACUL1 in breast cancer tissues have more negative emotions, lower social support level, lower quality of life and worse tumor staging.
