Abstract:【Objective】 To investigate the effect of dexmedetomidine(Dex) on analgesia, hemodynamics and postoperative recovery in patients undergoing orthopedics surgery with general anesthesia. 【Methods】 A total of 90 patients with spinal stenosis who underwent elective lumbar decompression and pedicle screw fixation plus bone graft fusion surgery were selected and randomly divided into two groups:, with 45 cases in each group. Before induction of anesthesia, patients in the observation group received intravenous infusion of Dex at 0.5 μg/kg, followed by 0.2 g/(kg·h) for maintenance, while patients in the control group was injected with the same volume of normal saline. The heart rate (HR), systolic blood pressure (SBP) and diastolic blood pressure (DBP) before anesthesia induction (T0), at anaesthesia induction (T1), 1 min (T2) and 5 min (T3) after endotracheal intubation, 1 min (T4) and 5 min (T5) after extubation of trachea, were recorded. The VAS scores at 2, 4, 8, 12, 24, and 48 hours after surgery, and the incidence of adverse reactions were compared in both groups. 【Results】 The HR, SBP and DBP values at T1 in the two groups were lower than those at T0 (P<0.05). The HR, SBP and DBP values at T2, T3 and T4 were higher than those at T1(P<0.05). The HR, SBP and DBP in the observation group were lower than those in the control group at T3, T4 and T5 (P<0.05). The VAS score, remifentanil dosage and 24 h morphine dosage in the observation group were lower than those in the control group at 2, 4, 8, 12, 24 and 48 h after operation (P<0.05). The incidence of adverse reactions such as cough, nausea, vomiting and respiratory depression in the observation group was lower than those in the control group (P<0.05). 【Conclusion】 Dexmedetomidine can maintain hemodynamic stability in patients with general anesthesia, reduce the dosage of anesthesia drugs and postoperative analgesic drugs, improve postoperative analgesic effect, and have less adverse reactions.
