摘要【目的】探讨创伤性脑损伤(TBI)患者血清ficolin-3水平与其损伤程度和预后的相关性。【方法】回顾性分析本院神经外科2015年8月至2016年8月收治的67例TBI患者临床资料,根据GCS评分将其分为中度组(中度脑损伤,GCS为9~12分, n =33)和重度组(重度脑损伤,GCS为3~8分, n =34例)。另选同期健康体检者40例为对照组。应用酶联免疫吸附法(ELISA)检测所有受试者血清ficolin-3水平。【结果】中度组、重度组受伤后24 h、48 h、72 h、120 h血清ficolin-3水平均显著低于对照组的(24.73±2.61)μg/mL,差异均有统计学意义(均 P <0.05)。在伤后24 h、48 h、72 h、120 h重度组血清ficolin-3水平均显著低于中度组,差异均有统计学意义( P <0.05)。在伤后各时间点,预后不良组患者血清ficolin-3水平均显著低于预后良好组,差异均有统计学意义( P <0.05)。TBI患者伤后24 h血清ficolin-3水平与GOS的Spearman相关分析显示,血清ficolin-3水平与GOS呈显著正相关(r=0.4855, P <0.05 )。ROC曲线分析显示,ROC曲线下面积(AUC)为0.743,血清ficolin-3水平临界值为14.39 μg/mL时,灵敏度和特异度分别为82.19%和60.03%。【结论】血清ficolin-3水平的降低在一定程度上可以反映TBI严重程度,可能是TBI引起的继发性脑损伤的潜在标志物,对TBI患者早期预后评估具有较高预测价值。
