Abstract:【Objective】To investigate the surgical outcomes of transcallosal transchoroidal approach and Krause approach to resect posterior third ventricular and pineal region tumors. 【Methods】Retrospective analysis of clinical data of patients with posterior third ventricle and pineal region tumors admitted to our hospital from March 2007 to October 2013 were studied, which included 54 cases of posterior third ventricular and pineal region tumors treated by the Krause approach and 20 cases of posterior third ventricular and pineal region tumors treated by the transcallosal transchoroidal approach. All the patients were assessed by MRI in less than 3 days after operation. 【Results】In the transcallosal transchroidal approach group, 20 patients underwent total resection of the tumor with 1 case (5.00%) coma due to frontal lobe hematoma. There were no deaths during surgery. However, there were 2 cases of postoperative hydrocephalus (10.00%) who underwent ventriculo-peritoneal shunt surgery. In the Krause approach group, 50 cases (92.58%) of posterior third ventricular and pineal region tumors were totally resected and 4 cases (7.4%) underwent subtotal resection, with 2 (3.70%) deaths and 4 (7.41%) comas. There were still 4 cases (7.41%) of hydrocephalus after operation, and endoscopic three-ventricle ostomy (ETV) and ventriculo-peritoneal shunt (VPS) were used.【Conclusion】 Both the transcallosal transchoroidal approach and Krause approach are good choices for pineal region tumors removal. The transcallosal transchoroidal approach is more appropriate for third ventricular tumor, while the Krause approach is more suitable for tumors located in the pineal region.
