Application of Laryngeal Mask Airway General Anesthesia Combined with Brachial Plexus Block Anesthesia in Upper Limb Surgery of Elderly Patients with Cardiovascular Diseases

Abstract:【Objective】To study the application value of laryngeal mask airway (LMA) general anesthesia combined with brachial plexus block anesthesia in upper limb surgery of elderly patients with cardiovascular diseases. 【Methods】A total of 74 elderly patients with cardiovascular diseases who underwent upper limb surgery were randomly divided into two groups with 37 cases each. Patients in the LMA group were given LMA general anesthesia combined with brachial plexus block anesthesia while patients in the control group were given general anesthesia by tracheal intubation combined with brachial plexus block anesthesia. The stress indexes, hemodynamics, dosage of narcotic drugs, recovery, pain and adverse reactions were compared between the two groups. 【Results】At the moment of tracheal/laryngeal mask extraction (T4), the levels of cortisol (Cor) and atrial natriuretic peptide (ANP) in the LMA group were significantly lower than those in the control group (P<0.05). Heart rate (HR) and mean arterial pressure (MAP) at T4 and T1 (tracheal/laryngeal mask intubation) were significantly lower than in the control group (P<0.05). The total incidence of adverse reactions in the laryngeal mask group was 16.2%, which was significantly lower than that in the control group (37.8%) (P<0.05). 【Conclusion】LMA general anesthesia combined with brachial plexus block anesthesia has the advantages of simple operation, little stress and stable hemodynamics. It can reduce the incidence of postoperative complications, and is suitable for elderly patients with cardiovascular disease.
