摘要【目的】探讨聚乳酸防粘连膜联合人工周期疗法在预防宫腔镜下宫腔粘连分离术(trans-cervical resection of adhesions,TCRA)后再粘连中的应用价值。【方法】选择2014年 9月至2016年9月50例中重度宫腔粘连在本院宫腔镜下行TCRA患者的临床资料,按不同的预防粘连方法将其分为聚乳酸防粘连膜包裹O型节育环联合人工周期疗法组(观察组,30例)和单纯置常用O型节育环联合人工周期疗法组(对照组,20例)。比较两组患者术后再粘连的预防效果。【结果】观察组在改善术后宫腔形态(93.33% vs 70.00%,P=0.0417)、月经情况表现(86.67% vs 60.00%, P=0.0375)、降低下腹痛(6.67% vs 30.00%,P=0.0338)、白带增多发生率(6.67% vs 35.00%, P=0.0201)均优于对照组,且其差异有统计学意义(P<0.05)。两组患者术后随访12个月,观察组在术后3、6、12个月等不同时间点粘连复发率明显低于对照组(P<0.05)。【结论】聚乳酸防粘连膜联合人工周期疗法在预防宫腔镜下TCRA术后再粘连的效果显著,安全性好,值得临床推广应用。
