鼻咽癌患者黏膜组织中CyclinD<sub>1</sub>和p16基因的表达及其临床意义

摘要【目的】探讨鼻咽癌(Nasopharyngeal Carcinoma,NPC)患者中CyclinD₁和p16基因的表达情况及其临床意义。【方法】选取2009年1月至2015年12月期间于本院就诊的未经放化疗治疗的70例NPC患者以及同期送检的55例慢性炎症患者鼻咽黏膜组织标本作为研究对象,采用免疫组化SP法测定其组织中CyclinD-1和p16蛋白表达,PCR检测p16基因缺失情况。【结果】NPC肿瘤组织中CyclinD-1蛋白的免疫组化评分及表达阳性率高于慢性炎症组织,而p16蛋白的免疫组化评分及表达阳性率低于慢性炎症组织,差异均具有统计学意义(P＜0.05);高等级TN分期及临床分期的患者CyclinD-1表达阳性率高于低等级TN分期及临床分期的患者的阳性率,而p16表达阳性率低于低等级TN分期及临床分期的患者的阳性率,差异均具有统计学意义(P＜0.05);NPC组患者p16基因缺失发生率(34.3％)高于慢性炎症组患者缺失发生率(1.8％),差异具有统计学意义(P＜0.05)。【结论】NPC组织中CyclinD-1高表达、p16基因缺失及p16低表达与NPC的发生以及预后具有密切关系。

Abstract：【Objective】To investigate the expression of CyclinD₁ and P16 in patients with nasopharyngeal carcinoma (NPC) .【Methods】A total of 70 patients with NPC and 55 patients with chronic rhinitis in our hospital from Jan. 2009 to Dec. 2015 were selected. The expression of P16 and CyclinD₁ protein in NPC and chronic rhinitis tissue were detected by SP immunohistochemistry, and the gene loss of p16 in patients of NPC was detected by polymerase chain reaction (PCR) .【Results】Positive expression rate of CyclinD₁ in the NPC group was higher than that of the chronic rhinitis group; and the difference between the two groups was statistically significant (P＜0.05) . Positive expression rate of P16 in the NPC group was lower than that of the chronic rhinitis group; and the difference was statistically significant (P＜0.05) . Positive expression rate of CyclinD₁ in the T₃~T₄ stage patients was significantly higher than that of the T₁~T₂ stage patients (P＜0.05) . Positive expression rate of CyclinD₁ in the N_1~3 stage patients was significantly higher than that of the N0 stages patients (P＜0.05) . Positive expression rate of CyclinD₁ in the Ⅲ~Ⅳ stage patients was significantly higher than that of the Ⅰ~Ⅱ stage patients (P＜0.05) . Positive expression rate of P16 in the T₃~T₄ stage patients was significantly lower than that of the T₁~T₂ stage patients (P＜0.05) . Positive expression rate of P16 in the N_1~3 stage patients was significantly lower than that of the N0 stage patients (P＜0.05) . Positive expression rate of P16 in the Ⅲ~Ⅳ stage patients was significantly lower than that of the Ⅰ~Ⅱ stage patients (P＜0.05) . The rate of p16 gene loss in NPC (34.3％) was significantly higher than that of the chronic rhinitis group (1.8%) ; and the difference was statistically significant (P＜0.05) .【Conclusion】Higher expression of CyclinD₁, the loss of p16 gene and the lower expression of P16 were associated with the occurrence and prognosis of NPC.
