Abstract:【Objective】To investigate the short-term efficacy of microscope assisted periodontal flap surgery in the treatment of moderate to severe periodontitis. 【Methods】From July 2013 to December 2014, the clinical data of 50 cases with moderate to severe periodontitis treated by microscope assisted periodontal flap surgery (study group) and 46 cases treated by periodontal flap surgery under common view (control group) were retrospectively analyzed. The oral health status [oral health index-simplified (OHI-S)]:Di-s and Ci-s, periodontal indexes probing depth [(PD), clinical attachment level (CAL), sulcus bleeding index (SBI)] were compared when patients were enrolled in the study (T0), 1 month (T1), 3 months (T2) and 6 months (T3) after surgery. The differences in pain scores [numerical rating scale (NRS)] and inflammatory factors [serum tumor necrosis factor α (TNF-α), interleukin -6 (IL-6), C-reactive protein (CRP)] before surgery, 24h and 72h after surgery were recorded. 【Results】At T1 to T3, the DI-S and CI-S indexes of the two groups were significantly lower than those of T0(P<0.05), but there was no significant difference between the groups at the same time point (P>0.05); PD, CAL and SBI with the two groups of patients were significantly lower than those of T0, and at the same time point, the observation group was significantly lower than the control group, the difference was statistically significant (P<0.05).At 24 h after operation, the scores of NRS in two groups were significantly higher than those before operation, but the score of observation group was lower than that of control group, the difference was statistically significant (P<0.05); At 72 h after operation, the scores of NRS in two groups were lower than those before operation and at 24 h after operation, and the score of observation group was lower than that of control group (P<0.05); At 24 h and 72 h after operation, the levels of TNF-α, IL-6 and CRP in the two groups were significantly lower than those before the operation, and the observation group was lower than that of the control group, the difference was statistically significant (P<0.05). 【Conclusion】Assisted periodontal flap surgery under microscope can effectively reduce the degree of pain in patients with moderate and severe periodontitis, improve the depth of periodontal pocket, reduce the loss of clinical attachment and relieve the bleeding of gingival sulcus, and is worthy of clinical application.
