Abstract:【Objective】To provide references for clinical administration, the excretion of cefathiamidine in patients with hysterectomy under lumbar-epidural anesthesia was studied.【Methods】Thirteen eligible patients with hysterectomy under lumbar-epidural anesthesia, ASA grade Ⅰ~Ⅲ, were selected. The operation lasted about 3 hours. Cefathiamidine was intravenously dripped in patients with hysterectomy under lumbar-epidural anesthesia for one hour; urine samples from 0~1 h、1~2 h、2~3 h、3~4 h、and 4~5 h after drug administration were collected and measured with HPLC-UV. WinNonlin6.1 software package was used for pharmacokinetic analysis.【Results】T1/2β in the lumbar-epidural anesthesia group was 1.95±0.97h, whereas T1/2β in the non-anesthesia group was (1.12±0.44)h. There was a significant difference between lumbar-epidural anesthesia group and the non-anesthesia group(P<0.05). The urinary average excretion rate of cefathiamidine detected within 0~6 hours showed that the peak was at 1.5 hours after the drug was administered in both the anesthesia and non-anesthesia groups. The average excretion rate of cefathiamidine in non-anesthesia condition was higher than that in anesthesia conditions. The average cumulative excretion rate of cefathiamidine in urine of the non-anesthesia group was significantly faster than that of the anesthesia group.【Conclusion】 T1/2β of cefathiamidine in lumbar-epidural anesthesia group was significantly higher than non-anesthesia group. This indicates that the body's stress response caused by anesthesia, perioperative medication, and surgical trauma could affect the excretion of drugs in the body and prolong the drug's half-life.
