Abstract:【Objective】To assess the clinical efficiency of T-SPOT.TB in the diagnosis of tuberculosis infection within infants. 【Methods】 A total of 118 infants (less than 3 years old) with respiratory infectious diseases were enrolled in this study and underwent T-SPOT.TB, tuberculin skin test (TST) and serum tuberculosis antibody (TB-Ab IgG)detections. All the patients enrolled were from Hunan Children's Hospital from February 2013 to September 2015. The specificity and sensitivity of the three methods were compared .The diagnostic value of T-SPOT.TB applied in determining active tuberculosis was then evaluated. 【Results】According to the diagnostic criteria, 58 infants with active tuberculosis were eligible for analysis and enrolled as a tuberculosis group, and 60 infants were enrolled in non-TB group. The sensitivity and specificity of T-SPOT.TB for the diagnosis of tuberculosis infection within infants were 89.6% and 93.3% respectively, higher than those of TST (83.6% and 66.7%) and tuberculosis antibody tests(TB-Ab IgG) (65.5% and 68.3%), the difference was statistically significant (P<0.05 or P<0.01). The positive predictive value and negative predictive value of T-SPOT.TB were also higher than those of the other two methods. 【Conclusion】Peripheral blood T-SPOT.TB shows a high sensitivity and specificity for the diagnosis of tuberculosis infection within infants, which may represent a promising and faster strategy for tuberculosis diagnosis.
