Abstract：【Objective】To evaluate the reasons why thyroid cancer was diagnosed as TI-RADS grade 2-3 by ultrasonography and to improve the diagnosis accuracy of atypical thyroid cancer and reduce unnecessary misdiagnoses. 【Methods】We retrospectively analyzed the clinic data and the ultrasound image characteristics of 30 thyroid cancers diagnosed as TI-RADS grade 2-3 by ultrasonography. 【Results】A total of 30 thyroid tumors that were diagnosed as TI-RADS 2-3 grade by preoperative ultrasound were pathological classified as 17 cases of papillary thyroid micro carcinoma,3 cases of papillary carcinoma, 6 cases of follicular carcinoma , and 4 cases of medullary carcinoma after surgery; out of these, Hashimoto's thyroiditis was found in 11 cases. 【Conclusion】The reasons for thyroid cancer to be misdiagnosed as TI-RADS 2-3 grade are lumps within the complicated thyroid background, micro-cancer lesions associated with multiple benign nodules, and single atypical thyroid medullary carcinoma manifestation. Therefore,atypical thyroid nodules need further fine needle aspiration.
