Study on the Gene Distribution and Relationship of Human Leukocyte Antigen (HLA) in Patients with Severe Skin Allergy Reaction Induced by Anti-Epileptic Drugs
Abstract：【Objective】To analyze the gene distribution of human leukocyte antigen (HLA) in patients with severe skin allergy reaction induced by anti-epileptic drugs. 【Methods】 Sixty patients with skin allergy reaction induced by anti-epileptic drugs who were treated between March 2013and March 2016 were selected as study subjects, including 35 patients with mild skin allergy reaction(mild group) and 25 patients with severe skin allergy reaction (severe group). At the same time, another 40 normal healthy persons were selected as control group. The distribution of HLA-B* 1502 in the three groups was determined by PCR-SSP method. The frequency of HLA-B* 1502, HLA-A* 1101、HLA-A* 3101 was compared between the three groups and the relationship between skin allergy reaction induced by anti-epileptic drugs and HLA genotype was analyzed. 【Results】The frequency of HLA-B* 1502 gene in the severe group (56.00%) was significantly higher than that in the mild group and the control group (11.43%, 7.50%) (P<0.05); Skin allergy reaction induced by anti-epileptic drugs was related to HLA-B * 1502, HLA-A * 1101, HLA-A * 3101 and HLA-B * 1511 (P<0.05), while mild skin allergy reaction had no significant correlation with genes mentioned above (P>0.05) (except for HLA-A * 3101). The severe skin allergy reactions in 25 cases were caused by the aromatic anti-epileptic drugs. 【Conclusion】 The main HLA genes of severe skin allergy reaction induced by anti-epileptic drugs are HLA-B * 1502 and HLA-B* 1511. Severe skin allergy reaction induced by anti-epileptic drugs is significantly related to positive HLA-B * 1502 and other genes. The detection of HLA-B *1502 and other genotypes is suggested before anti-epileptic drug therapy. If it is positive, aromatic antiepileptic drug treatment ought to be avoided.
