Abstract:【Objective】To investigate the safety and clinical effect of OneStage approach combined posterior and anterior intervertebral autogenous iliac bone graft for the treatment of lumbar tuberculosis in children.【Methods】13 cases of patients from 7 to 14 years old ( average age about 11.5) have received preoperative anti tuberculosis treatment ever lasted 2 to 4 weeks before the OneStage operation: posterior pedicle screw fixation, anterior debridement of tuberculosis focus, intervertebral autogenous iliac bone graft. All patients had 6 to 8 weeks postoperative bed rest plus antituberculosis treatment lasted 12~18 months.【Results】The period followup was 20~28 months and all operation incisions healed up primarily, without neurological progress and sinus tractor fistulae formation. The height of vertebral body and the correction of the posterior deformity were satisfactory, and there was no displacement of bone graft, and the patients have solid bony fusion after 4 to 7 months. 6 cases with neurological symptoms had recovered with neither kyphosis deformity progress nor internal fixation loosening nor rupture after operation. The tuberculosis foci were healed up, there was no obvious abnormality of the vertebral body, and no obvious abnormality was found in the pelvis.【Conclusion】The One Stage approach combined posterior and anterior with intervertebral autogenous iliac bone graft proved to be a safe and effective method for the treatment of children's lumbar tuberculosis, which can abolish tuberculosis foci , fully decompress for spinal cord, effectively correct the deformity of the spine and prevent the development of the spine deformity.
