摘要【目的】探讨微炎症状态因子高敏C反应蛋白(hs-CRP )、白细胞介素-6(IL-6)、肿瘤坏死因子-α(TNF-α)对糖尿病肾病(DN)早期的诊断意义。【方法】选择了77例按1999年WHO诊断标准诊断为2型糖尿病T2DM的患者,再根据尿清蛋白排泄率(UAER)分为三组:糖尿病(DM )正常清蛋白尿组(NDM组,UAER<30 mg/24 h);早期DN组(EDN组,UAER介于30~300 mg/24 h ,持续性的微量清蛋白尿);临床DN组(CDN组,UAER>300 mg/24 h ,持续性大量清蛋白尿)。另选择健康体检正常者33例作为对照组(CON组)。对所有受检者上述微炎症因子进行检测。【结果】hsCRP检测浓度EDN组和CDN组均显著高于CON组和NDM组( P<0.01);IL-6检测浓度EDN组和CDN组均显著高于CON组( P <0.01);TNF-α的检测浓度三个实验组均显著高于CON组( P<0.01),三个实验组两两之间比较存在显著性差异( P <0.01)。【结论】DN患者早期微炎症状态因子hs-CRP、TNF-α、IL-6水平升高,提示DM患者肾病早期即存在炎症。
Abstract:To explore the significance of micro-inflammatory state factors high-sensitivity C-reactive protein(hs-CRP) ,interleukin-6(IL-6) and tumor necrosis factor-alpha(TNF-α) in the diagnosis of ear-ly diabetic nephropathy(DN) .[Methods]A total of 77 patients diagnosed as type 2 diabetes mellitus(T2DM ) according to the WHO criteria in 1999 were chosen .According to urinary albumin excretion rate (UAER) ,all patients were divided into normal UAER group (NDM group ,UAER< 30mg/24h) ,early DN group (group EDN ,UAER = 30-300mg/24h ,continuous micro-albuminuria state) and clinical DN group (group CDN , UAER>300mg/24h ,continuous great albuminuria state ) .Other 33 healthy subjects were chosen as group CON .Above micro-inflammatory factors in all subjects were measured .[Results] The levels of hs-CRP in group EDN and group CDN were significantly higher than those in group CON and group NDM ( P <0 .01) . The levels of IL-6 in group EDN and group CDN group were significantly higher than those in group CON ( P<0 .01) .The levels of TNF-αin 3 experimental groups were significantly higher than those in group CON ( P<0 .01) ,and there were significant differences among 3 experimental groups( P <0 .01) .[Conclusion] The early micro-inflammatory state factors hs-CRP ,TNF-αand IL-6 in DN patients are elevated ,which indicates that inflammation occurs in early nephropathy of diabetic patients .
